If your daughter requires prescription eyewear and cannot comfortably wear standard lacrosse goggles, there is an option to apply for a waiver to allow her to wear prescription sports goggles that meet specific requirements. Here’s everything you need to know to get started:

Step-by-Step Waiver Process

  1. Download the Necessary Forms:

    • Please download both the Waiver and Doctor’s Note from the attachments below. Important: Do not send in any additional medical information or forms.

  2. Complete the Forms:

    • Fill out the forms thoroughly and ensure that both you and your doctor sign them.

  3. Submit the Forms:

  4. Receive the Signed Waiver:

    • After we review and process your waiver, we’ll sign it and return it to whoever sent it to us. The coach will need to keep a copy of this waiver with them at every game throughout the season, in case referees request to see it.

Important Information:

  • This waiver applies only to players aged 14 and under who require prescription eyewear.

  • Only specific eyewear models that meet ASTM F3077-17 standards and SEI certification are allowed. Please refer to the waiver for a list of approved models.

  • We recommend options like the Bangerz OTG goggles (designed to fit over glasses) or RecSpecs by Liberty Sports Slam goggles (available with prescription lenses). Another brand to consider is Betty; however, we are less familiar with their products.

  • This waiver process must be completed annually, so please ensure to submit it each year if your daughter continues to need prescription eyewear.
